Note: If you have more than 5 provider investments in your portfolio, please print an additional form to complete.  If you have joint investments in your portfolio please complete a separate form for each owner of the investments and duplicate the joint owned investments on each form, signing where stated.

The form really is as simple as it looks.  All you need to do is list any holdings you have, (ISA’s, PEP’s, Unit Trusts, Investment Trusts, OEICS or Investment Bonds) and then sign and date it, ensuring you include the provider of the investment on the form.

This instructs your fund manager to give us all the necessary up-to-date information and tells them to deal with us as your servicing agent.

Everything to gain, nothing to lose

If your existing broker is not supplying good, or indeed, any comment, register your holdings with our portfolio Healthcheck to receive our views.  There are no costs and no changes to your holdings so what have you got to lose?
